「健康自家管」長者慢性病管理服務義工訓練班
香港長者患有慢性疾病情況普遍，但部分長者礙於身體情況，不便外出到醫院或者中心學習有關的自我管理知識，以致病情惡化。所以香港紅十字會由2014年起開展「健康自家管」長者慢性病自我管理服務，透過「到戶式」服務，由個案經理及註冊護士上門評估、義工定期上門探訪，主要從身體、心理及社交三個層面推動及協助長者自我管理慢性病。
有興趣的民眾可加入義工訓練班，掌握家居探訪及溝通技巧後，在上門探訪、電話慰問等，能為長者帶來健康知識之餘，更送上一份關懷同愛心。
